Problem

The wife contacted our elder care services company because her husband has dementia and behavioral issues. We met with her 4 months ago while her husband was still at home with her, and we had concerns for both his safety as well as her own. She was not certain as to how she would physically be able to transition her husband into a licensed care community. We explained that not if, but when her husband should have a decline in health and need hospitalization, that would be a window of opportunity for transitioning him quickly into a licensed care environment.

Solution
Her husband, did in fact, have a decline in his health and was admitted to the hospital via 911. Fortunately, she remembered our initial conversation, had him admitted to the hospital, and after a 3-day stay he was admitted to a skilled rehab facility in Williamsburg.

The wife then contacted us to provide her with geriatric case management assistance. We were able to:

• Assist with the discharge from the skilled rehab facility
• Assist with the selection of an appropriate, specialized memory care community
• Set up the admission screening
• Provide ongoing case management to both the institutionalized spouse as well as the wife to ensure a
smooth transition and adjustment into the specialized memory care community
